Event Evaluation Form

Please provide constructive feedback to help organizers improve our events. Your comments and email address go only to the Area Chair.  The Area Chair will send your comments, but not your email address, to the Organizer.  If you have a serious problem with the judges or technical delegate, please fill out the USEF licensed judging evaluation form.  If you want the USEA to tabulate your evaluation, go here.
